About The Position

As a Psychogeriatric Case Manager, you will be part of a team that co-ordinates a support network and linkages for older adult and seniors with serious mental illness and other challenges (e.g., addictions). A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity, and self-respect. The role will require direct service and support to clients in area such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs. Crisis Outreach Services for Seniors (COSS) is an on-call mobile crisis service for older adults (65+ years) who are frail, socially isolated, at-risk of repeated hospital use and/or elder abuse due to living with mental health and substance use challenges. The COSS team delivers services within 24-48 hours either in the individuals own home or in their local community. Short-term crisis response is available 365 days a year. The team provides crisis counselling, short-term intensive case management, harm reduction, mental health and addictions support, health assessment and care as need by a Nurse Practitioner, and referral to primary care. COSS is a partnership with Woodgreen and Reconnect ensuring that all of Toronto has access to this service. LOFT’s COSS team supports those individuals living within the northern Toronto geographical area.

Requirements

  • Completion of a diploma/degree in Social Services/Human Services or equivalent
  • Minimum three years related experience in providing support to vulnerable seniors and older adults with responsive behaviours related to mental illness, addictions, dementia or other neurological conditions
  • Group counselling related to mental health, addictions, and related issues.
  • Knowledge of community resources and support
  • An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with client on goals/priorities.
  • Crisis intervention skills, with well-developed problem solving abilities and interpersonal skills
  • Good oral and written communication skills and computer literacy, including MS word and Excel.
  • Counselling, responding and assessment skills.
  • Creativity and flexibility to approaches.
  • Ability to work effectively as part of a team and interact with workers from a variety of agencies.
  • Understanding of cultural competency and framework
  • Possession of a valid driver’s license, and a reliable vehicle in order to travel is required.
  • A satisfactory vulnerable sector check (VSC) is also required.

  • Completion of Gerontology is an asset.
  • Ability to communicate in a different language, an asset.

Responsibilities

  • Assists persons served in the development and implementation of a care plan tailored to the needs of the person served and issues designed to improve quality of life and assist in the mitigation of responsive behaviours.
  • Identify and refer to resources specific to those needs and issues.
  • Act as a liaison and advocate for the person served among those involved with the individual’s care including family, other care-partners, medical and other multi-disciplinary providers.
  • Complete assessment such as RAI and OCAN and provide timely and accurate record keeping and reports as required.
  • Provide supportive counselling or referral and linkages to health care, programs or service providers.
  • Provide crisis management/intervention and assist in the development of crisis plans for persons served.
  • Develop respectful and trusting relationships with persons served using Recovery and Psychosocial Rehabilitation principles.
  • Maintain strict confidentiality and follow all other required policies and procedures.
  • Consistently interact with persons served, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization and the Behavioural Supports Ontario Project.
  • Effectively participate as a member of the team; attend staff/team meetings, care conferences for person served, other multi-disciplinary meetings, committee meetings etc. as required.
  • Attend and actively participate in all required training.
  • Liaise, collaborate and coordinate with other professionals, service providers and partners within the Behavioural Support Services.
  • Complete other duties, from time to time, as assigned by the Program Manager and/or Associate Director.
