About The Position

Provide case management and case coordination to families and single individuals. This role involves working with various housing authorities and community partners to support program participants in achieving housing stability and accessing necessary services.

Requirements

  • Associates Degree in Counseling, Human Services, Sociology or Social Work. May consider at least 2 years’ experience in case management, crisis intervention or counseling.
  • Experience and proficiency in database management and Microsoft Office applications.
  • A valid and clean NYS driver’s license.
  • Must have a reliable vehicle and be willing to transport program participants.

Nice To Haves

  • Bilingual in Spanish is preferred.

Responsibilities

  • Work with Rochester Housing Authority to complete intakes and initial screenings.
  • Provide case management and coordination services, including consistent contact with head of household, entitlements assistance, and support for medical, mental health, employment, literacy, higher education, legal services, safety planning, and goal planning.
  • Develop and maintain working relationships with landlords and property management, establishing consistent communication to support households and address barriers to housing stability.
  • Develop YWCA and community partnerships beneficial to program participants.
  • Enter timely and accurate data into HMIS (Homeless Management Information System) per coordinated access and YWCA policies.
  • Develop goals, document all contact, income changes, and outcomes in HMIS.
  • Maintain household files per YWCA and funder standards.
  • Communicate and case conference regularly with supervisor and other outside organizations working with program participants.
  • Link participants to appropriate services and activities within and outside the YWCA as identified in the program participant individualized service plan.
  • Assist program participants with the re-certification process as needed.
  • Case conference with Rochester Housing Authority, Partners Ending Homelessness, property rental manager, providers, and other community resources.
  • Participate in community workgroups/meetings related to the Continuum of Care and coordinated access.
  • Demonstrate understanding of and sensitivity to cultural differences, person-centered practices, and trauma-informed care.
  • Demonstrate compliance with all appropriate HIPAA Privacy and Security Standards.
  • Attend monthly department and all-staff meetings.
  • Perform other duties as requested.
