About The Position

Wellspring is seeking a full-time Case Manager (PAD) to assist clients in gaining access to needed medical, psychiatric, substance use disorder services, mainstream benefits, obtaining IDs, housing, peer recovery services, social, employment and other services while helping clients successfully navigate the criminal legal system. The Case Manager (CM) develops and revises comprehensive needs assessments with individualized care plans. The CM refers and links the individual to needed services, monitors and provides follow-up regarding the efficacy of services and advocates for services/service quality.

Requirements

  • A Bachelor’s or Master’s in Criminal Justice, Psychology, Sociology, Social Work, Human Services, or related degree.
  • One year of full-time employment working directly with adults with a serious mental illness (SMI) and/or with justice-involved adults.
  • Strong ability to establish and maintain an effective working relationship with adults with a serious mental illness, colleagues and agency/community providers.
  • Knowledge of recovery-based case management for adults with SMI and Co-occurring Disorders.
  • Consistently demonstrates a strong respect for the dignity of all individuals.
  • Working knowledge of community resources and ability to facilitate coordination activities.
  • Clearances of state and federal background checks, multiple abuse/sex offender registries, and clearance of exclusion lists prior to employment.
  • Successfully passing a pre-employment drug screening.
  • Access to a personal, registered, and insured vehicle to transport clients and to travel between client locations and related appointments.
  • Possess a valid, active driver's license in good standing.
