Requirements

  • Bachelor's degree in Human Services, Social Work, Psychology, Counseling, or a related field OR High School Diploma/Associate Degree with at least 3 years of relevant experience working with youth, families, or behavioral health populations
  • Current CDCA (Chemical Dependency Counselor Assistant) credential required
  • Current QMHS (Qualified Mental Health Specialist) credential required
  • Compassionate and trauma-informed
  • Excellent communicator and active listener
  • Skilled at building trust and engagement
  • Organized and able to manage multiple priorities
  • Creative problem-solver who thinks critically under pressure
  • Comfortable navigating social service, healthcare, and community systems
  • Proficient with Microsoft Office and case management software

Nice To Haves

  • Experience providing mental health case management services
  • Experience working with youth and families in community-based, school-based, or behavioral health settings
  • Knowledge of local community resources and support systems
  • Outstanding communication and relationship-building skills
  • Strong written documentation abilities
  • Excellent organizational and time management skills

Responsibilities

  • Build strong, supportive relationships with youth, families, and community partners.
  • Advocate for clients and empower them to achieve their personal goals.
  • Serve as a trusted guide throughout the treatment and transition process.
  • Help individuals identify and access community supports, services, and opportunities.
  • Partner with schools, employers, healthcare providers, and community organizations.
  • Remove barriers that impact education, employment, and successful community living.
  • Assess client needs and help determine the most appropriate level of care.
  • Coordinate individualized service plans, treatment goals, and discharge planning.
  • Support clients in developing daily living skills, independence, and self-sufficiency.
  • Assist with crisis intervention, stabilization, and ongoing support when challenges arise.
  • Maintain accurate documentation and client records.
  • Complete progress notes and service documentation in a timely manner.
  • Ensure compliance with agency standards and regulatory requirements.
  • Work closely with the treatment team to coordinate quality care and positive outcomes.
  • Begin discharge planning from day one.
  • Help clients successfully transition back into their communities.
  • Provide follow-up support after discharge to encourage continued success and stability.
