About The Position

The HeartShare St. Vincent’s Services (HSVS) Nuevo Camino program provides a supportive and nurturing environment for 38 unaccompanied minors, ages 5-12. The program offers transitional foster homes, daily educational services Monday through Friday, and wrap-around services in Brooklyn, NY. The program is committed to trauma-informed, culturally appropriate, and needs-tailored service delivery, while adhering to all Office of Refugee Resettlements (ORR) program requirements. The Nuevo Camino program is seeking a case manager to oversee and support these young persons. The case manager will identify appropriate sponsors (parents or guardians) for the child's release and unification. They will assess the safety and well-being of potential sponsors, identifying and discussing any concerns with the Nuevo Camino team and Federal Field Specialist. Additionally, the case manager ensures the child receives essential services including medical and dental care, mental health services, basic education, orientation, and legal screening.

Requirements

  • Bachelor's degree from a four-year college or equivalent required.
  • 1 year of similar experience required.
  • Bilingual and fluent in English and Spanish required.
  • Strong communication and writing skills.
  • Ability to work collaboratively with clients and co-workers.
  • Knowledgeable about working with immigrant youth population.
  • Superior organizational and time management skills.
  • Ability to work independently and as part of a team.
  • Ability to manage time effectively, multi-task, and determine priorities.
  • Knowledgeable about ACS/OCFS regulations governing disciplinary actions.
  • Experience and facility working with Microsoft Office Applications.
  • Ability to work flexible hours as needed.
  • Proof of vaccination prior to start date required.
  • CPR-Certified a plus.

Responsibilities
