About The Position

The Case Manager is responsible for providing case management services to consumers of the Day Treatment Program. This role works closely with the Director and Supervisors to ensure the agency’s mission of providing individual counseling, group therapy, and crisis intervention for persons is adhered to. Services are provided within a drug-and violence-free Day Treatment Program. The Case Manager is responsible for adhering to the Office of Mental Health (OMH) and the agency’s policies and procedures, and for bringing all significant concerns regarding the program and/or consumers to the Clinical Director.

Requirements

  • Must be able to relate effectively to mandating agencies, evaluators, regulatory and other agencies.
  • Must be computer literate and have excellent interpersonal, written and verbal communication skills.
  • Must have strong leadership qualities and be a team player.
  • Must be capable of handling routine and crisis situations using good judgment and direction.
  • Must be able to travel independently.
  • Must be able to sit approximately six hours a day.
  • Must enforce the policies and procedures that govern this agency.
  • Regular attendance is required.
  • Must be available to work a flexible schedule including some evenings, weekends and holidays.
  • Employment is contingent upon the successful completion of a background check and/or fingerprinting (department-dependent), which will be conducted following a conditional offer of employment in adherence to NYC Fair Chance Act guidelines.

Nice To Haves

  • Minimum of one year experience working with MICA population.
  • Working knowledge of addiction counseling.
  • Working knowledge of the regulations that govern MICA/Day Treatment Programs.
  • Masters Degree in Social Work or Mental Health Counseling is preferred

Responsibilities

  • Facilitates therapeutic groups as assigned, documenting results in group notes and any significant issues in progress notes.
  • Facilitates weekly caseload groups where treatment plan goals are addressed, entering results in progress notes.
  • Attends bi-weekly interdisciplinary staff meetings and presents case presentations.
  • Is able to de-escalate crisis or special situations.
  • Is available to lead or assist in special groups such as money management or art therapy, as assigned.
  • Conducts individual counseling sessions once every two weeks with each person on caseload, documenting results in the clinical folder. The content of counseling sessions must relate to treatment plan goals and meet OMH and OASAS regulations.
  • Ensures that all incidents that occur during program hours are reported to their direct supervisor.
  • Monitors and ensures the consumer’s personal hygiene and ADL Skills are developed in a consistent and satisfactory manner. These issues must be addressed in groups and individually.
  • Observes and relates to consumers on a daily basis and provides feedback to the treatment team regarding behaviors and problems. Notes such matters in the daily communication log.
  • Participates in scheduled staff development meetings and scheduled staff training sessions.
  • Attends regular clinical supervision.
  • Participates in the identification of health and legal problems of the consumers and communicates in a timely manner such information to the treatment team.
  • Regular attendance is required.
  • Performs other job/assignments requested by Executive staff members.
