About The Position

The Masters of Social Work/Licensed Clinical Social Work (MSW/LCSW) Case Manager serves as a leader of the complex discharge planning process and a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive, complex, and safe discharge plans. Collaborates with RN Case Manager’s for guidance with medically complex cases. Handles complex cases requiring advanced social work knowledge. The MSW/LCSW Case Manager is also responsible for facilitating coordination of care. Provides guidance and shares expertise with other case managers, leads, and conducts training sessions on new resources and programs. The MSW/LCSW Case Manager along with the other members of the healthcare team accepts responsibility, authority, and accountability for management of care This shall be in accordance with the current policies and procedures, mission, vision, and values of UVACH. Serves as a resource for discharge planning and care coordination for psychosocially complex cases. Maintains current knowledge of available community resources and post-acute care options.

Requirements

  • Master's Degree in Social Work from school accredited by the Council on Social Work Education.
  • Licensed Clinical Social Worker (LCSW) Certification required.
  • Weekend and holiday coverage.

Nice To Haves

  • 2 or more years of professional social work experience or 2 or more years of healthcare experience preferred.
  • Case Management or related experience preferred.
  • Knowledge/Competence of Microsoft Office products and keyboarding.
  • Ability to handle high-stress situations effectively
  • Maintains confidentiality in all matters related to patient care and visitor issues.
  • Provides cultural information to facilitate support for a treatment plan.
  • Ensures that information pertaining to the patient's outpatient services, hospitalization, (i.e., advance directives, consents from treatment, diagnosis and prognosis, treatment plan, follow-up appointments and Health Care Education) are accurately communicated.
  • Empathy and the ability to build rapport with patients and families
