The Key to Home Case Manager will be a team member in a two-year multi-organization project that focuses on in person, housing-based case management services. As a team member, the case manager will assist in successfully housing and stabilizing a minimum of 40 households during the two-year funding period. The essential job duties include assisting clients in encampments and remaining with clients for a year after they are housed, completing timely HMIS program enrollments, assisting clients with timely completion of unit and voucher applications, and assisting with timely move-ins. The role also involves providing one year of weekly in-person, home-based case management services, including intake/assessment, long-term housing stabilization planning, budget planning, opportunities to increase income, assistance connecting to behavioral health and physical healthcare services, and eviction prevention. The Case Manager will also record case documentation, participate in daily encampment huddles and weekly/monthly stabilization work group meetings, liaise with other service providers and community partners, act as a point of contact for various teams, adhere to Coordinated Entry Policies and CoC Standards of Care, participate in the CoC, and demonstrate flexibility, consistency, and kindness. Providing transportation to clients as needed is also a responsibility.
