Lutheran Services Florida (LSF) is seeking a talented Case Manager - Intensive to provide trauma-informed intensive case management and supportive services to vulnerable clients, helping them achieve their goals. This role involves conducting weekly in-person trauma support, crisis intervention, safety planning, and family preservation. The Case Manager will also provide client-centered ongoing intensive case management for vulnerable clients with special circumstances, such as medical or psychological vulnerability, family conflict, or education-related issues. Key responsibilities include conducting comprehensive intakes, developing goal-oriented self-sufficiency plans, maintaining communication with supervisors, utilizing trauma-focused interventions, providing cultural orientation workshops, guiding participants through referrals, screening for financial assistance, collaborating with other case managers, and guiding volunteers. The role also involves participating in community coordination, maintaining comprehensive case files, understanding programmatic requirements, attending stakeholder meetings and trainings, supervising interns, and preparing reports.
