About The Position

This role involves completing initial and ongoing assessments by gathering information from children, their families, and collaterals. The Case Manager may be responsible for initial contact of children and subsequent placement, researching and recommending kinship placements, and facilitating family meetings. A key aspect of the role is assisting youth with the development and implementation of transition plans, and developing and implementing case plans and permanency goals with children and families, ensuring goals are met within established time frames. The Case Manager determines and recommends necessary services, including mental health, drug and alcohol services, respite care, and supervised or unsupervised visitations. They also assist youth in establishing support networks, prepare youth for transitions from the foster care system, and provide crisis intervention. The position requires the ability to work with the court system, including preparing reports, participating in court proceedings, and ensuring court orders are followed. Accurate and timely documentation in case records is essential. The Case Manager supports kinship and foster families, participates in supervisory and team meetings, and collaborates with family support workers. Supervision and transportation of children and families may be required. The role includes an on-call rotation for urgent issues outside regular working hours, requiring availability, rapid response, problem resolution, potential short-notice travel, and thorough documentation. Participation in group or individual supervision is also part of the role.

Requirements

  • Bachelor's degree in social work or human service field from an accredited college or university.
  • Must be 21 years of age.
  • Must pass a drug screen, MVR, KBI, Child Abuse and Neglect Central Registry Clearance check and an Adult Registry Check.
  • FBI fingerprint check.
  • Lifting Requirements of 50 lbs.
  • Must have a valid driver's license, acceptable driving record and reliable transportation.
  • Mandated reporter.
  • Receives training in trauma informed practice and incorporates knowledge in day-to-day practice.
  • Ability to work flexible schedule including on-call.
  • Understanding of family centered practice.
  • Understanding of the child welfare and state system.
  • Ability to engage families in working relationships.
  • Understanding of child and adolescent development
  • Demonstrates effective written and verbal communication skills.
  • Ability to travel throughout the State.
  • Consistently exercises discretion and independent judgment in performance of duties in conformance with applicable policies, procedures, statutes, rules, and regulations.

Responsibilities

  • Determines and recommends which services are needed for the child and family, including mental health, drug and alcohol services, respite care and supervised or unsupervised visitations/interactions.
  • Assists youth to establish a support network of family, kinship, and community.
  • Prepares youth for moves and transition from the foster care system.
  • Provides intervention for youth in crisis situations.
  • Ability to work with the court system and follow court protocols including appropriate business attire.
  • Completes reports for court, participates in court proceedings, including testifying, and makes sure court orders are followed.
  • Completes and maintains written documentation in child/family's case record accurately and within established time frames.
  • Supports kinship and foster families to maintain placements and help youth work toward permanency.
  • Participates in supervisory and team meetings, sharing pertinent information about children/families' progress, critical trends, barriers and solutions to achieving outcomes; and relationships with other agencies/communities.
  • Works collaboratively and provides guidance to family support workers assigned.
  • Supervises and transports children and families as needed.
  • On-call rotation where you will be available outside of regular working hours to address any urgent issues that arise during your rotation.
  • Participates in group or individual supervision.
