About The Position

The SILP Case Planner is responsible for all aspects of case management services for a caseload of youth residing in the SILP Program. The case planner provides concrete services, resources, counseling and is an integrated member of the SILP team which strives to ensure improved outcomes for youth. The SILP Case Planner will provide services while residents are in care and provide aftercare services upon discharge. The SILP Case Planner is responsible for all paperwork required by ACS; OCFS and the courts.

Requirements

  • BA/BSW degree from an accredited college.
  • Experience working with adolescents.
  • At least three years of case work/case planning related experience.
  • Understanding of the critical nature of timely and accurate documentation of service delivery and case work practice.
  • Knowledge and experience in child development as well as adolescent development.
  • Ability to be a team player.
  • Ability to work independently.
