About The Position

The Mobility Mentoring Program is based on a research-informed approach to help families improve their well-being, meet their children's needs, and get out of poverty. This model is particularly relevant to child welfare, given that scarcity is a major underlying factor for many families receiving child welfare prevention services. It promotes financial management, education, employment, family stability, and mental health. Services include individual and family counseling, domestic violence counseling, case management, behavior management, school advocacy, home visits, referral services, parent skills training, and more.

Requirements

  • Bachelor’s Degree required
  • Knowledge of Motivational Interviewing and coaching practices.
  • Ability to work with adults towards economic mobility goals such as educational advancement, career enhancement and financial literacy.
  • Knowledge of best practices in the field and connections with systems addressing the aforementioned areas.
  • Excellent organization skills, willingness to report on all aspects of work and analyze efforts for continuous improvement.
  • Must be able to manage a caseload of up to 12-13 families at a minimum effectively and efficiently.
  • Ability to work flexible hours (evenings/Saturdays) as assigned.
  • Able to conduct home/field visits and travel throughout the five boroughs as needed.
  • Ability to lift up to 10lbs.

Nice To Haves

  • Experience in case management is preferred.
  • Experience engaging with child- and family-serving systems in the greater New York area is preferred.
  • Experience working with individuals who have experienced trauma and crises such as homelessness, domestic violence, poverty.

Responsibilities

  • Develop psycho-social assessments and service plans for children and families to ensure child safety and reduce child risk in accordance with the Improved Outcomes for Children (IOC) initiative.
  • Perform case management duties to a caseload minimum of 12 including advocacy and assistance with entitlements.
  • Participate in Family Team Conferences (FTC’s) and complete Family Assessment Service Plans (FASP’s) for designated caseload.
  • Complete progress notes both in the Connections system and offline on template forms.
  • Complete monthly casework statistics using the PROMIS system.
  • Participate in various staff meetings.
  • Provide one to one mentoring, coaching and goal setting, specifically around goals related to the promotion of economic mobility and increased family alignment for the head of household and her family, including children.
  • Serve as a resource for the participant and family on issues including housing, budgeting, career and education, childcare, health care, and more.
  • Conduct detailed assessments, monitor and document participants’ progress according to individual plans.
  • Recruit participants for the program, including outreaching to community groups, presentations to local agencies, etc.
  • Thorough and organized documentation of caseload efforts, including up to date and accurate documentation of electronic database system according to deadlines.
  • Participate in the development and facilitation of participants’ workshops and community groups.
  • Complete initial and ongoing needs assessments to determine the individual’s most appropriate level of care management.
  • Facilitate solutions to each family’s service delivery problems and identify with client any potential barriers to care.
  • Responsible for the overall management of the family’s Individualized Service Plan.
  • Monitor and evaluate a client’s needs, including prevention, wellness, medical, mental health, care transitions, and social and community services where appropriate.
