About The Position

As a member of the team, it is your opportunity to play a vital role in providing wellness coaching, care coordination, and support navigating services to children, youth, and families. You will help connect individuals to essential resources and care, making a meaningful impact every day. You will deliver ongoing community-based assessment, treatment, and supports for children and youth experiencing or at-risk for severe emotional disturbance, assisting them and their families as they traverse healthcare, community systems, and education systems.

Requirements

  • Bachelor's degree in related field
  • Experience with providing support to parents around a myriad of childhood issues including but not limited to, development, behavioral management, and attachment
  • Experience interfacing with community partners
  • Ability to work cooperatively and responsibly as a member of a team from diverse backgrounds
  • Abide by confidentiality policies
  • Due to the rural nature of our catchment area, a valid driver’s license, excellent driving record and access to a safe, reliable, insured vehicle is required

Responsibilities

  • Identify youth and family needs, strengths, and goals while simultaneously addressing barriers to accessing services
  • Make referrals and follow up on services
  • Support communication between providers and members of the youth’s team
  • Support youth with goal-setting, organization, problem-solving, and self-advocacy
  • Provide support in schools, homes, and community settings
  • Document services, referrals, coordination, and progress
  • Collaborate with clinicians and other CYFS staff
