About The Position

Since 1952, Light of Life Rescue Mission has been a safe refuge for homeless and hurting people in the Pittsburgh area. We currently seek a full-time Case Manager (Level 2) for the Women & Children's Longterm Program. This person will walk closely alongside clients in guiding them through a four-phase recovery program that combines clinical and spiritual elements. Successful people in this role tend to be persuasive problem-solvers who are skilled in building relationships and inspiring others.

Requirements

  • Bachelor's degree required in social work, counseling, psychology, human services, or another related field.
  • 5-years of experience working with individuals in a substance recovery or mental health setting.
  • Excellent customer service experience.
  • Valid driver's license and clean driving record.
  • Proficient in Microsoft Office.

Nice To Haves

  • Master's degree is preferred.
  • HMIS experience and knowledge preferred.

Responsibilities

  • Provide case management and advocacy for clients, working as a liaison to coordinate wraparound services, while making referrals for housing, healthcare, and other resources for families.
  • Develop individualized treatment plans addressing substance use and co-occurring disorders and teach families about addiction, recovery, trauma, and effective support strategies.
  • Act as a liaison between medical and social services providers as needed.
  • Maintain detailed, confidential client records and case notes.
  • Track clients' care plan progress through one-on-one discussions and using the online client management system, MissionTracker.
  • Provide immediate support during emergencies and connect clients to appropriate crisis services.
  • Network with related service providers to access appropriate services for clients.
  • Serve as a treatment team member, providing input and support in Care Team decisions.
  • Make decisions about proposed activities in accordance with Light of Life principles and the best practices for recovery.
  • Transport clients in the Mission's vehicles to and from appointments as needed.
  • Work in tandem with the children's case manager and volunteers.
  • Walk alongside clients as a disciple of Christ, sharing the Gospel through actions and words.
  • Establish mutual trust and create a safe, authentic environment.
  • Serve clients and co-workers in a loving manner and practice a self-evaluative lifestyle.
  • Resolve all issues in ways that promote the Mission's core values while following the processes described in the staff handbook.
  • Help the clients make choices that are justifiably in their eternal best interest.
  • Practice Christ-like stewardship of all resources.
  • Model personal responsibility, accountability, timely reporting, and a self-evaluative lifestyle.
