About The Position

People Incorporated is seeking a Case Manager II to join our Waiver Case Management - St. Louis County (Metro based) team. This role involves working with clients diagnosed with chronic mental illness or other ongoing health conditions. The primary focus is connecting people with services by developing person-centered Support Plans and authorizing services that promote independent living and community integration. Waiver Case Management emphasizes behind-the-scenes service coordination. Case Managers typically meet with clients twice annually, fostering long-term relationships through dynamic service plan development. Staff operate autonomously, creating their own hybrid schedules after initial training. The position offers a strong work-life balance with manageable caseloads averaging 30 individuals.

Requirements

  • Must reside within the metro area
  • Valid driver’s license, excellent driving record and current auto insurance
  • Bachelor’s Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field; OR Bachelor’s Degree from an accredited four-year college with a major in any field, AND One year of experience as a social worker/case manager/care coordinator in a public or private social service agency

Nice To Haves

  • Experience providing direct services to adults with serious and persistent mental illness
  • Experience with case management and commitment proceedings
  • Knowledge of Recovery, Motivational Interviewing and Person-Centered approach
  • Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions, AND One year of experience in the education or treatment of persons with developmental disabilities or related conditions

Responsibilities

  • Attending bi-annual meetings with clients to monitor services
  • Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications
  • Assessing client needs and ensuring continuity of care.
  • Maintain regular contact with collaborative partners.
  • Making referrals and authorizing services based on client needs and personal outcomes.
  • Maintaining records and documenting all services provided.
  • Excels at documenting all interventions, interactions, and relevant information to the Support Plan consistent with professional and organizational standards
