The primary focus is connecting clients with services by developing person-centered Support Plans and authorizing services to promote independent living and community integration. Waiver Case Management emphasizes behind-the-scenes service coordination, with Case Managers meeting clients twice annually to develop long-term relationships and dynamic service plans. Staff operate autonomously, creating their own hybrid schedules after initial training. The position offers manageable caseload sizes, averaging 30 individuals, and promotes work-life balance. The role is full-time, Monday-Friday, with business hours from 8 am to 4:30 pm.
