About The Position

People Incorporated is seeking a Case Manager II to join our Waiver Case Management team in St. Louis County. This role involves working with clients diagnosed with chronic mental illness or other ongoing health conditions. The primary focus is connecting clients with services by developing person-centered Support Plans and authorizing services to promote independent living and community integration. Waiver Case Management emphasizes behind-the-scenes service coordination, with Case Managers meeting clients twice annually to develop long-term relationships and dynamic service plans. Staff operate autonomously, creating their own hybrid schedules after initial training. The position offers manageable caseload sizes, averaging 30 individuals, and promotes work-life balance. The role is full-time, Monday-Friday, with business hours from 8 am to 4:30 pm. Requirements

  • Must reside within the metro area
  • Passing all background and requirements for facility and position
  • The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
  • Must have a valid and current driver’s license and auto insurance.
  • Valid driver’s license, excellent driving record and current auto insurance
  • Bachelor’s Degree from an accredited 4-year college with a major in social work, psychology, sociology or a closely related field; OR Bachelor’s Degree from an accredited four-year college with a major in any field, AND One year of experience as a social worker/case manager/care coordinator in a public or private social service agency

Nice To Haves

  • Experience providing direct services to adults with serious and persistent mental illness
  • Experience with case management and commitment proceedings
  • Knowledge of Recovery, Motivational Interviewing and Person-Centered approach
  • Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions, AND One year of experience in the education or treatment of persons with developmental disabilities or related conditions

Responsibilities

  • Attending bi-annual meetings with clients to monitor services
  • Develop and maintain a collaborative relationship with clients serving as a role model for clients by demonstrating positive, proactive and respectful communications
  • Assessing client needs and ensuring continuity of care.
  • Maintain regular contact with collaborative partners.
  • Making referrals and authorizing services based on client needs and personal outcomes.
  • Maintaining records and documenting all services provided.
  • Excels at documenting all interventions, interactions, and relevant information to the Support Plan consistent with professional and organizational standards
