About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Telecare Los Angeles Service Area 4 (LA 4) serves 212 adults (ages 18+) and has three voluntary programs: Assertive Community Treatment (ACT), Full-Service Partnership (FSP) and Outpatient Community Services (OCS).

Requirements

  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or Associates degree and one (1) year of experience; or Bachelor’s degree in Social Services
  • Valid and current driver’s license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
  • Willingness to use the company vehicle to drive clients to appointments and groups, etc.

  • Full-Time | AM | Shifts: 8:00 AM - 4:30 PM | Days: Monday - Friday
