About The Position

The Mental Health Case Manager PROMISE is responsible for interacting with clients, staff, and outside contacts with respect and courtesy, while maintaining client and staff confidentiality. This role involves attending staff meetings, supervisory conferences, and training sessions. The case manager will orient and assess new clients, complete and update Service Plans with client input, and monitor client progress according to funding source standards. Responsibilities include scheduling and facilitating case management meetings, identifying and developing resources, and linking clients to needed services. The position also requires facilitating scheduled workshops/programs/groups, fulfilling training agreement obligations, and developing appropriate discharge plans. Additional duties include facilitating house meetings, participating in Treatment Team meetings, completing required reports and documentation, demonstrating competency in program-related certifications, and providing support to the program assistant function. Specific requirements include obtaining LSI-R Training and Certification within 90 days of hire, attending court or parole hearings as needed, facilitating at least one core curriculum program, and conducting drug and alcohol screens. The role also involves delivering specialized mental health treatment interventions and performing other assigned duties.

Requirements

  • Master’s Degree preferably in Social Work (MSW), or a Master’s degree in Counseling or another Human Services related field.
  • Minimum of two (2) years professional experience in mental health counseling.
  • Minimum of two (2) years professional experience working with special needs populations.
  • Practical skills in communication.
  • Practical skills in counseling processes.
  • Practical skills in resource development.
  • Working with addictive personalities.
  • A basic understanding of human behavior.
  • Utilizing group dynamics to address special needs populations.
  • Effective written and verbal communication skills.
  • Ability to interact appropriately with clientele.
  • Ability to pass criminal background clearance from funding source.
  • Valid driver's license and ability to meet agency driver standards.
  • Must be able to facilitate at least one core curriculum program.
  • Must obtain LSI-R Training and Certification within 90 days of hire.

Nice To Haves

  • Basic computer skills are preferred.
  • Bilingual is a plus.

Responsibilities

  • Interact with clients, staff, and outside contacts with respect and courtesy.
  • Maintain client and staff confidentiality in compliance with administrative policies and procedures.
  • Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions.
  • Orient and assess all new clients via the approved agency assessment tools.
  • Complete Service Plans on all assigned clients based on approved agency assessment tools with client input.
  • Monitor and document client progress according to funding source standards and/or operational policy and procedures.
  • Update service plan with client input.
  • Schedule and facilitate case management meetings with clients as required.
  • Identify and/or develop resources and link clients to needed services.
  • Facilitate scheduled workshops/programs/groups addressing client's needs.
  • Fulfill obligations of training agreements entered into for core curriculum programs.
  • Develop appropriate discharge plan on all assigned clients as per contractual requirements.
  • Facilitate scheduled house meetings as needed.
  • Participate in required internal/external Treatment Team meetings.
  • Complete all required reports and other documentation.
  • Demonstrate competency in program related certifications.
  • Provide support to program assistant function.
  • Obtain LSI-R Training and Certification within 90 days of hire.
  • Attend court hearings and/or parole hearings as subpoenaed or assigned.
  • Facilitate at least one core curriculum program.
  • Conduct drug and alcohol screens as directed or needed and report results to Director or Manager.
  • Deliver specialized mental health treatment interventions to address risk/needs.
  • Other duties as assigned.
