Provides case management services to patients assigned to individual caseload. Intensive Case Manager roles often involve working with unhoused individuals with mental health challenges, substance use disorders, or complex trauma. The core of the job is establishing a trusting relationship with the individual, often through a person-centered, motivational interviewing approach. The case manager conducts initial assessments, identifies individual needs, and links them to appropriate services, including mental health care, medical care, substance abuse treatment, legal aid, and benefits enrollment. A significant part of the role involves assisting with housing applications, securing rental assistance, and providing ongoing support to help individuals maintain their housing.
