About The Position

CRS provides coordination, support, services, and establishment of the referral and screening procedures process. The Case Manager I function as an advocate and support person to those individuals assigned to their assigned program and links them to appropriate services and monitors the progress or lack of progress by the individual and his or her family. The Case Manager I will work closely with the individual, the individual’s family, LifeStream staff, and other community organizations to meet their needs and to allow the individual to live as independently as possible within the community.

Requirements

  • Bachelor’s degree with major course work in the human services field required
  • Minimum of 1 year of experience in case management, social services, or a related field.
  • Experience working with diverse populations, including individuals with mental health or substance abuse issues.
  • Strong understanding of case management principles and practices.
  • Excellent communication skills, both verbal and written, with the ability to interact effectively with clients, families, and professionals.
  • Ability to assess client needs and develop appropriate service plans.
  • Proficient in maintaining detailed and accurate records and documentation.
  • Strong organizational skills with the ability to manage multiple cases simultaneously.
  • Demonstrated ability to work independently and as part of a team.
  • Familiarity with community resources and ability to connect clients with appropriate services.
  • Competence in using case management software and Microsoft Office Suite.

Responsibilities

  • Conduct thorough assessments to identify client needs, strengths, and goals.
  • Develop, implement, and monitor individualized service plans in collaboration with clients and their families.
  • Coordinate and facilitate access to community resources, services, and supports.
  • Advocate for clients' needs and rights within the healthcare and social service systems.
  • Maintain accurate and up-to-date documentation of client interactions, progress, and outcomes.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support for clients.
  • Provide crisis intervention and support as needed.
  • Educate clients and their families about available resources and services.
  • Monitor and evaluate the effectiveness of service plans and make adjustments as necessary.
  • Participate in training and professional development opportunities to enhance skills and knowledge.
