About The Position

The Case Manager will be responsible for Homeless Outreach and Case Management Services, conducting various job duties. The Case Manager will provide client services and resource referrals as needed.

Requirements

  • Bachelor’s degree in the Human Service Field
  • One-year experience working with homeless population and/or individuals with disabilities is required
  • Time management and multi-tasking skills are a must
  • Have valid driver’s license and current vehicle insurance

Responsibilities

  • Conduct street outreach as scheduled by the Program Director
  • Conducting Street Outreach to fulfill a contract with HUD
  • Responsible for linking participants to referrals and community services in EBR
  • Complete intake and initial assessment.
  • Data entry into HMIS (Homeless Management Information System)
  • Inputting Coordinated Entry Data
  • Driving the mobile outreach van
  • Assist program individuals in obtaining community support, resources, and services
  • Complete monthly and quarterly program reports
  • Follow all guidelines & procedures related to EZ Labor (clocking in & out, reviewing time cards, requesting edit sheets, etc)
  • Report any significant concerns to the supervisor
  • Maintain training requirements
  • Projects and duties as assigned
