About The Position

Buckelew Programs is a leading provider of behavioral healthcare services in the North Bay, offering compassionate mental health, supported housing, and substance use treatment. This full-time Case Manager I position involves supporting clients in achieving greater independence and stability while living in the community. The role requires working collaboratively within a multidisciplinary team to provide services in psychiatric rehabilitation, medication and symptom management, relapse prevention, harm reduction, independent living skills, wellness coaching, housing support, budgeting, resource linkage, and community integration. These duties apply across all agency programs unless otherwise specified.

Requirements

  • Associate degree and six or more years of experience in mental health, physical restoration, social adjustment, or vocational adjustment (for Fully Qualified MHRS).
  • Bachelor’s degree and four or more years of experience in mental health, physical restoration, social adjustment, or vocational adjustment (for Fully Qualified MHRS).
  • Master’s degree with two or more years of related experience (for Fully Qualified MHRS).
  • Fluent in English.
  • Ability to work independently with strong organization, flexibility, empathy, and sound judgment.
  • Demonstrated professionalism and the ability to maintain healthy boundaries.
  • Strong interpersonal skills with the ability to relate to clients, colleagues, and external partners.
  • Intermediate proficiency in Microsoft Office (Excel, Word, Outlook).
  • Valid California driver’s license.
  • Insurable driving record.
  • Access to an insured vehicle required.
  • Must pass Live Scan background check (as required by some programs).
  • Ability to intermittently perform physical tasks such as bending, stretching, squatting, reaching, lifting (up to 30 lbs.), and driving.
  • Extended periods of sitting, typing, writing, standing, and driving may be required.

Nice To Haves

  • Bilingual in English/Spanish.
  • Experience with Electronic Health Record (EHR) systems.
  • Master’s degree or higher (for Case Manager II, preferred for the field).
  • Experience with individuals with mental health and/or substance use disorders (for Case Manager II, preferred for the field).
  • Licensure (MFT, LCSW, etc.) or progress toward licensure (for Case Manager II, preferred for the field).

Responsibilities

  • Meet agency billing and documentation standards within required timeframes.
  • Maintain accurate, timely, and complete client records and data tracking related to performance outcomes.
  • Communicate promptly and professionally with internal teams and external providers via phone, email, and in-person contact.
  • Develop, implement, and monitor individualized service plans that align with program philosophy and client goals.
  • Support clients with daily living skills such as meal planning, cooking, shopping, and household tasks, and teaching skills where needed.
  • Observe and report changes in client behavior, mood, and functioning.
  • Maintain awareness of cultural and physical/mental health needs.
  • Promote adherence to safety protocols and model appropriate safety practices.
  • Provide individual and group counseling, case management, and skill development that fosters recovery, responsibility, and positive social engagement.
  • Assist clients with system navigation (e.g., medical, psychiatric, legal, or detention facilities) and linkages to community support like peer-based groups (e.g., AA, NA, Wellness Centers).
  • Provide crisis intervention and management in coordination with supervisory or licensed staff.
  • Facilitate groups, classes, or community-based activities as directed.
  • Monitor and support medication adherence, including prompts, refills, proper storage, and reporting per program guidelines.
  • Participate in program planning and represent the team on agency committees as assigned.
  • Provide transportation using personal or agency vehicles and assist with the use of public transportation systems.
  • Ensure clients attend to all medical and related appointments.
  • Perform minor repairs at agency sites or residences; report major repair needs to the supervisor promptly.
  • Ensure all services meet agency quality standards and support measurable client progress and satisfaction.
  • Maintain compliance with all regulatory, legal, and contractual requirements (e.g., CARF, Medi-Cal, HUD), including licensing and program certifications.
