About The Position

This position is located in Wagoner, Oklahoma. The Case Manager I/II role involves extensive travel and requires a valid driver's license and maintained car insurance. All new employees will complete the Case Manager Academy Training Program. The role focuses on providing services to individuals with intellectual and/or developmental disabilities and their families, ensuring compliance with federal and state regulations.

Requirements

  • Bachelor's degree in a human services field OR a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing.
  • For Case Manager Level I: One (1) year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field.
  • For Case Manager Level II: Two (2) years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field.
  • Valid driver's license.
  • Must maintain required car insurance.

Responsibilities

  • Assures agency and community resources to meet consumer's needs, serves as QIDP
  • Provides a monthly monitoring service of residential and vocational services
  • Manages and prioritizes a caseload and provides services to individuals with intellectual and/or developmental disabilities and their families.
  • This position is required to meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines.
