About The Position

The Case Manager I, Community Programs, develops and implements a person-centered program for all assigned clients to achieve their individual goals, and provides intensive support services to develop the abilities necessary to maximize their independent functioning in a work and community environment.

Requirements

  • A Degree in a Human Services field is preferred.
  • Experience working with individuals with intellectual and developmental disabilities is preferred.
  • Medication Administration Qualification (or complete training within 30 days).
  • CPR/First Aid training or willingness to obtain training.
  • Excellent written and verbal communication skills are preferred.
  • Demonstrate the ability to interact with various types of internal and external customers is preferred.
  • Ability to relate effectively to disabled persons, their parents, and guardians and be empathetic toward their needs is preferred.
  • Ability to work independently, cooperatively, and to exercise effective planning.
  • Demonstrate flexibility within the framework and the pressure of established deadlines.
  • Ability to write clear, concise, and accurate reports and plans, submit them in a timely fashion.
  • Possess excellent time management skills.
  • Working knowledge of community resources.
  • Ability to program, train, and be creative in motivating clients toward individual success.
  • Conduct oneself in a professional manner and maintain a professional appearance that enhances the firm’s reputation.
  • Experience speaking effectively to various small groups in a presenting/informative role.
  • Overtime may be required as well as some travel to attend meetings and required training.
  • Ability to drive for company business is preferred.
  • Submit to a criminal background check.
  • Report any arrests, charges, or summonses for any disqualifying offense as specified by C.R.S. §27-90-111 to the State.
  • Be at least 21 years of age (not engaged in passenger transportation) for inclusion on the Goodwill driver insurance schedule.
  • Be at least 25 years of age for all passenger transportation services.
  • Be at least 25 years of age for CDL.
  • Undergo a Motor Vehicle Record (MVR) check and background check.
  • Disclose all moving traffic violations or vehicle crashes (within the last five (5) years).
  • Must have valid Colorado State driver license.
  • Must also be able to operate company vehicle, and adhere to all Federal, State, and local laws governing vehicle operation.
  • Must have valid vehicle insurance, valid driver license, and vehicle must be in roadworthy condition if using personal vehicle for company business.

Responsibilities

  • Provide direction, supervision, and case management for all assigned clients.
  • Complete an initial person-centered plan and assessment, develop and implement an individual program plan, and document progress for the client.
  • Act as the Chair for annual staffing meetings of unfunded clients and function as part of the Interdisciplinary Team to plan and develop the person-centered program.
  • Plan ancillary activities for clients to include special staffing and initiate appropriate referrals for special services when necessary.
  • Encourage independence by assisting clients in problem-solving their personal and work-related problems by using accepted and innovative rehabilitation techniques and maintaining a proactive environment.
  • Teach proper work habits, interaction skills, and self-help skills through role modeling and programmatic intervention.
  • Assist with medication administration, assist with assigned consumer transportation by completing all consumer pick-ups and drop-offs, and assist in any needed personal care of the consumer/client.
  • Be an active member of the team and assist team members with all client care as needed, including Activities for Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
  • Assist clients with ADLs such as transferring, mobility, eating, toileting, administering medications, and administering gastrostomy tubes (g-tubes) services.
  • Assist clients with IADLs such as managing transportation, lunch preparation and set up, and managing communication.
  • Provide supervision at the beginning and end of the day to ensure the safety of clients when both exiting and entering the facility and transportation.
  • Assist clients during lunchtime, as needed, to ensure protocols are followed thoroughly.
  • Complete all necessary documentation, e.g. incident reports, program plans, charting, file reviews and final reports.
  • Maintain complete and accurate records and statistical data as required for all programs.
  • Communicate and coordinate with other involved agencies and support personnel, e.g., residential staff, Case Management Agency, parents and guardians, The Department of Vocational Rehabilitation, and schools.
  • Keep the Program Coordinator informed of progress, considerations, and pertinent information.
  • Deal effectively and calmly with non-routine situations following accepted guidelines.
  • Perform other duties as required by the Supervisor III, Program Coordinator.
  • Research and keep updated by reading trade journals as needed to be aware of trends, theories, or initiatives within Case Management and other applicable within the industry.
  • Seek out seminars and symposiums for ideas and stay current with trends in new software, tools, and resources available.
  • Maintain a working and current knowledge of 1205 Rules and Regulations, and Standards of the CARF and a good working knowledge of Goodwill of Colorado’s policies and procedures.
