About The Position

Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Telecare's program in Santa Clara County is a 100-member high-fidelity Assertive Community Treatment (ACT) program for adults and older adults with severe and persistent mental illness (SPMI), co-occurring substance use issues, and /or other challenging and complex needs that increase their risk for poor treatment outcomes. Wraparound services and supports will include clinical interventions, such as multidisciplinary assessment and treatment, team-based case management, 24/7/365 crisis intervention, medication support, and integrated co-occurring substance abuse services, as well as non-mental health support in areas such as community integration, housing, employment and education, transportation, and benefits.

Requirements

  • High School Graduation or G.E.D. equivalent and three (3) years of direct services in mental health or community services; OR Associate’s degree and two (2) years of experience; OR Bachelor’s degree in Social Services
  • At least 1 year of direct service experience providing assistance as a Housing Specialist
  • Valid and current driver’s license
  • Personal vehicle insurance with your name listed as a driver
  • Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
  • Willingness to use the company vehicle to drive clients to appointments and groups, etc.
  • Willingness to use your personal vehicle to attend meetings, etc.

Responsibilities

  • Serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders.

  • Provides assistance as a Housing Specialist.
  • Drives clients to appointments and groups using a personal or company vehicle.
  • Attends meetings using a personal vehicle.
