About The Position

As a Housing Case Manager (Scattered Site), you walk alongside individuals and families who are navigating homelessness and working toward housing stability. Through consistent, relationship‑based case management, you help clients access housing, strengthen life skills, and remain stably housed in the community. This role is ideal for someone who believes housing is a human right, values trauma‑informed and harm‑reduction approaches, and wants to do meaningful work in a mission‑driven organization. Avivo is a 32‑hour work week organization, offering full‑time pay and benefits while supporting balance, sustainability, and wellbeing.

Requirements

  • High school diploma or GED
  • One year of experience working with adults experiencing homelessness or similar populations OR an associate degree or higher (no experience required)
  • Lived experience may be considered as part of the experience requirement
  • Strong relationship‑building and communication skills
  • Ability to work with individuals facing complex barriers with compassion and accountability
  • Interest in housing‑first, trauma‑informed, and harm‑reduction practices
  • Basic comfort with documentation and learning multiple data systems
  • Reliable transportation, a valid driver’s license, and ability to travel within the community
  • Flexibility to meet client needs, including occasional evenings or on‑call coverage

Responsibilities

  • Build trusting, person‑centered relationships with individuals and families experiencing homelessness
  • Maintain an assigned caseload and meet with clients regularly through in‑person and creative outreach
  • Complete strength‑based assessments and partner with clients to create housing‑focused goal plans
  • Support clients while they are in shelter or temporary settings to prepare for housing opportunities
  • Assist clients in locating, obtaining, and maintaining scattered‑site housing
  • Coordinate services related to medical care, mental health, substance use, income, and benefits
  • Support housing stability through landlord communication, eviction prevention, and mediation when needed
  • Help clients understand tenant rights and responsibilities
  • Document services and housing activities accurately and on time in required systems
  • Collaborate closely with teammates, supervisors, and community partners
  • Practice Avivo’s person‑centered, trauma‑informed, harm‑reduction, and anti‑oppression approaches
