About The Position

Housing is the foundation for stability — and your role helps make that possible. As a Case Manager with the Housing Advocacy & Resource Team (HART), you will partner directly with individuals and families experiencing homelessness to support their journey toward stable housing and long‑term success. Through relationship‑based work, you will help participants navigate complex systems, secure housing, and maintain stability in the community. This role is ideal for someone who believes in Housing First, harm reduction, and person‑centered care, and is motivated to walk alongside people through complex challenges with dignity and respect. Requirements

  • High school diploma or GED
  • One or more years of experience working with individuals experiencing homelessness or similar populations OR an associate degree or higher (no experience required)
  • Lived experience may be considered toward experience requirements
  • Strong relationship‑building skills with the ability to engage individuals in a non‑judgmental, person‑centered way
  • Understanding of or willingness to learn Housing First, harm reduction, and trauma‑informed care
  • Ability to manage multiple priorities, documentation, and timelines in a fast‑paced environment
  • Strong communication and problem‑solving skills
  • Knowledge of community resources or ability to learn quickly
  • Valid driver’s license, reliable transportation, and ability to travel within the community
  • Flexibility to meet program needs, including occasional evenings or on‑call coverage

Responsibilities

  • Build strong, trusting relationships with participants through consistent, person‑centered engagement
  • Maintain an active caseload and meet regularly with clients using creative and flexible outreach approaches
  • Complete assessments and develop individualized, housing‑focused goal plans with participants
  • Support individuals and families in locating, obtaining, and maintaining stable housing
  • Coordinate care and connect participants to medical, mental health, substance use, and community resources
  • Collaborate with landlords and mediate concerns to support housing stability
  • Proactively address risks to housing, including eviction prevention and problem‑solving strategies
  • Support participants in understanding tenant rights, responsibilities, and independent living skills
  • Document services, contacts, and outcomes accurately and on time in required systems
  • Work collaboratively with team members, supervisors, and community partners to support client success
