About The Position

This position will be responsible for providing case management services to residents of The Dorms Residential Center for Homeless Veterans. The Case Manager will complete an initial assessment for every resident within prescribed timelines; will develop a plan of care for each resident with a goal of returning that resident to self-sufficiency and independence. The Case Manager will coordinate with other agencies to ensure the delivery of needed services to residents and will work closely with residents to monitor progress toward reaching mutually established goals.

Requirements

  • This position requires a minimum of a Bachelors of Sociology, Psychology, and experience in clinical counseling.
  • The candidate should preferably have at least 4 years of increasingly responsible experience with the provision of services to veterans and/or the homeless.
  • Knowledge of veteran’s services and programs is critical.
  • Prior military experience is desirable.
  • Must have transportation and a valid Texas driver’s license.
  • Must also provide proof of current vehicle liability insurance.
  • Must pass a drug test and background record check.

Responsibilities

  • Treats each resident respectfully.
  • Provides new residents with an initial orientation and review Resident Handbook and Center operations with each new arrival.
  • Completes initial assessment of all new residents in a timely manner
  • Develops an Individual Service Strategy Plan for each resident
  • Coordinates needed care for each resident with appropriate agencies
  • Maintains case files and other documentation in an up-to-date manner
  • Completes reports and other documentation in a timely and accurate manner
  • Monitors the progress of residents toward established goals.
  • Provides assistance, as needed, to residents in obtained benefits for which they are eligible
  • Periodically evaluates each resident's readiness for independent living
  • Supervises Practicum students, as assigned
  • May participate in preparing documents, reports, literature, or presentations.
  • Must understand the nuances of the veterans’ community and are expected to always demonstrate sensitivity and patience with the clients.
  • Confidentiality of client information, donors, team members and the AGIF-NVOP. Such information is to be disclosed on a need-to-know basis only.
  • Responsible for being sensitive to the needs of program participants and must always treat clients with respect and professional care.
