About The Position

The Case Manager - Fatherhood PRIDE is responsible for community outreach to recruit fathers for the Fatherhood PRIDE program, enrolling them, providing case management services, facilitating workshops, and collecting evaluation data. This role involves engaging fathers, monitoring their progress, connecting them with community resources, and maintaining accurate records. The position also includes planning and coordinating program events and administrative duties. This role requires a commitment to the organization's vision and mission, and the ability to work independently and as part of a team.

Requirements

  • Bachelor’s Degree in social services or related field or equivalent work experience.
  • Four or more years of professional experience in community services and/or counseling with high-risk populations in the non-profit sector (may substitute for degree).
  • Ability to communicate effectively orally and in writing with individuals from diverse socio-economic backgrounds.
  • Excellent computer proficiency (MS Office Word, Excel and Outlook).
  • Ability to work under pressure and meet deadlines while maintaining a positive attitude and providing exemplary customer service.
  • Knowledge of the criminal justice system and child support court systems.
  • Ability to work flexible traditional and non-traditional hours.
  • Ability to establish and maintain effective working relationships with Coalition partner agency staff and the general public.
  • Ability to conduct discussions, interviews, and meetings.
  • Ability to maintain professional boundaries.
  • Experience in making public presentations and public speaking.
  • Ability to maintain confidentiality.
  • Knowledge of counseling principles and record-keeping techniques.
  • Knowledge of community resources.
  • Experience working with persons in crisis.
  • Ability to work independently and carry out assignments to completion.
  • Must be able to communicate effectively and be a team player.
  • Must be able to commit to the Coalition vision and be passionate about its mission.
  • Requires reliable transportation, valid driver’s license, good driving record, and automobile insurance.
  • Ability to safely and successfully perform essential job functions.
  • Must be able to travel to alternate locations.
  • Regularly required to sit, stand, walk, speak, and hear.
  • Sufficient hand dexterity to use a computer keyboard.
  • Capable of reading a computer screen.
  • Lifting/carrying up to 25 pounds.

Responsibilities

  • Conduct community outreach to recruit fathers for the Fatherhood PRIDE program.
  • Enroll fathers into the program and provide case management services.
  • Facilitate fatherhood workshops, including topics like 24/7 Dad, Inside Out Dad, Boot Camp for New Dads, Mom’s Gateway, and Growing Responsible Fathers.
  • Coordinate workshop schedules, locations, guest presenters, and materials.
  • Provide onsite support to outside facilitators for financial literacy and workforce development.
  • Ensure fathers complete the intake process and program requirements.
  • Develop and monitor weekly case plans, including goal setting and progress monitoring.
  • Engage regularly with fathers to ensure completion of program requirements (parenting, financial literacy, workforce development, case management).
  • Practice effective father engagement techniques and promote father friendliness.
  • Maintain caseload to meet enrollment and completion numbers.
  • Connect fathers with needed community resources and follow up on referrals.
  • Input accurate and complete data into the agency database.
  • Ensure validity of all submitted documents and provide required documentation in a timely manner.
  • Follow data collection protocols for reporting and input data within prescribed timeframes.
  • Plan, coordinate, and implement quarterly gatherings for fathers.
  • Plan, coordinate, implement, and attend special events like the Father’s Day Barbecue competition, Barbers for Babies, and Infant Mortality Awareness events.
  • Attend staff and team in-services, conferences, workshops, and community meetings.
  • Participate in grantee initiatives.
  • Promote community awareness regarding the value of involved fathers.
  • Conduct outreach to promote workshop schedules and father attendance.
  • Assist in identifying, locating, and engaging partners to refer fathers.
  • Network within the community to provide case management resources for fathers.
  • Maintain relationships with current partners.
  • Perform other duties as assigned by the Director, Fatherhood Project.
