About The Position

The Jamaica Plain Neighborhood Development Corporation (JPNDC) is a grassroots community development corporation with 48 years of experience in community organizing and resident leadership. JPNDC aims to transform the lives and amplify the voices of Boston residents who have been excluded from prosperity due to an inequitable economic system. They achieve this by building affordable housing, providing economic prosperity services to help individuals build on their strengths, and organizing people to pursue desired changes. The Case Manager will be part of the Economic Prosperity Services (EPS) team, reporting to the Senior Case Manager. The EPS case management team offers intensive support to residents of JPNDC's affordable housing properties through the RAMWP program. This support includes individual case management meetings, collaboration with the property management team, and community events at the properties. The Case Manager will also assist the EPS Family Mobility Services team by providing short-term case management support to individuals in the greater Boston community. This is a full-time position with a flexible schedule that includes some evenings and weekends, and frequent site visits. The role is suited for individuals passionate about making a positive community impact by supporting and stabilizing individuals facing challenging circumstances.

Requirements

  • Bachelor's degree in social sciences and related fields or relevant experience
  • At least one year of experience providing intensive case management (defined as long-term service delivery, weekly meetings with clients on caseload)
  • Experience engaging in assessment and service planning for clients
  • Experience with documentation of client progress
  • Experience with Trauma informed care
  • Flexibility and adaptability in work
  • Professional written and verbal communication
  • Organized, strong attention to detail, and solution-oriented

Nice To Haves

  • Strong crisis management skills
  • Experience with HUD funded housing
  • Navigation of mass housing assistance programs like RAFT
  • Ability to navigate multiple flights of stairs regularly to access client homes and buildings without elevator access

Responsibilities

  • Provide individualized case management from a whole family perspective, including assessment, service planning, implementation, and progress monitoring.
  • Develop and follow individualized service plans created with clients.
  • Work with residents and clients facing complex challenges such as rental arrears, income instability, benefits loss, mental and physical health conditions, and other barriers to stabilization.
  • Coordinate and engage in transition meetings with other departments within JPNDC.
  • Be available to assist with isolated short-term needs for residents in addition to longer-term clients.
  • Work from multiple job sites and meet participants in their homes, if needed.
  • Attend Housing Court when needed, as assessed by the program supervisor.
  • Maintain strong working knowledge of community, city, and state resources and connect clients directly to services including employment, education, legal assistance, housing support, childcare, financial benefits, and emergency resources.
  • Serve as a liaison with external service providers and collaborate on case planning, referrals, and follow-up.
  • Encourage program participation through various outreach methods including flyering, presentations, events, and networking.
  • Coordinate community-building events with residents.
  • Develop materials to market events and provide resources to clients.
  • Provide occasional group workshops.
  • Keep ongoing records of interactions in a timely and professional manner.
  • Track changes and progress through case planning and assessment data.
  • Process referrals through multiple domains.
  • Uphold confidentiality, professionalism, trauma-informed, and culturally responsive practice at all times.
  • Perform clerical tasks such as photocopying, scanning, filing, faxing, and printing.
  • Participate in required training and professional development.
  • Perform related duties as assigned by supervisor.
