This position is responsible for conducting comprehensive assessments of clients' needs, participating in the development of service plans, and arranging for the delivery of needed services. The Case Manager will assist clients in accessing services, monitor their progress, and perform advocacy activities. This role involves preparing and maintaining case records, providing case consultation, and communicating with supervisors regarding case management issues. Additionally, the Case Manager will provide psychosocial education to clients and families, perform crisis assistance, and may serve as a Primary Case Coordinator (PCC).
