At Optimal Case Management, we believe meaningful case management begins with listening. Our Coordinators of Community Services (CCS) partner with individuals with intellectual and developmental disabilities and their families to understand what matters most to them, navigate services and resources, advocate for their needs, and support progress toward meaningful life goals. This is rewarding, relationship-centered work that also requires independence, organization, accountability, travel, timely documentation, and consistent follow-through. We want candidates to understand both the purpose and the day-to-day realities of the position before joining our team. This is a community-based position—not a traditional remote job. Administrative work may be completed from a home-based workspace, but regular in-person community work is an essential part of the position. You will routinely travel throughout your assigned service region. Meetings may occur in individuals' homes, provider locations, workplaces, programs, and other community settings. Travel varies according to caseload needs and may occasionally involve driving up to approximately two hours one way. Applicants are strongly encouraged to apply for positions within or near your residence. You must have reliable transportation and the ability to travel throughout your assigned region.
