The Case Manager - Clinic plays a crucial role in client support within a community-based setting. Responsibilities include client enrollment in programs, needs assessment, implementation of evidence-based parenting and family strengthening curriculums, care plan development, service coordination, and follow-up. The role also involves data entry into various databases, participation in program outreach, and community networking. Direct service treatment encompasses diagnostic intakes, needs assessment, concrete service application, and ongoing care planning for individuals, families, and groups. The position requires caseload management, information/referral advocacy, community outreach, presentations, workshops, and parent education training. Generating and implementing Individual Service Plans and other case planning activities are key. The role involves participation and presentation in case reviews, consultations, and staff meetings, as well as assisting in program start-up and statistical/recording activities. Services are provided throughout Riverside County. The position also entails compilation of statistics, report preparation, and managing funding/contract source relations. Punctual and regular attendance is expected, along with other assigned duties.
