Join Our Team as a Case Manager (Bilingual)! Are you passionate about supporting Behavioral Health & Justice Involved Population and making a real difference in their lives? EAC Network, a leading not-for-profit social service agency dedicated to helping over 79,000 people through 100 impactful programs across Long Island and NYC, is looking for a dedicated full time Bilingual Case Manager to join the Nassau Family Treatment Court Expansion program.

Requirements

  • Bachelor’s degree in Social or Behavioral Science, Criminal Justice or CASAC and minimum of two years’ related experience.
  • Good communication skills, both oral and written.
  • Fluency in the Spanish language required.
  • Ability to work cooperatively with treatment and Family Court System.
  • Must have valid NYS (New York State) Driver’s License and be willing to transport clients to appointments using own vehicle.
  • Must provide proof of valid NYS car insurance and Defensive Driving Certificate within 30 days of hire.
  • Valid registration, inspection and Automobile Insurance Liability of a recommended minimum of 100,000/300,000 will also be evaluated upon hire.
  • Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, email and/or database software necessary to perform work assignments.

Responsibilities

  • Act as liaison to the Family Court System.
  • Conduct screening and assessment interviews and arrange for client to be placed in appropriate treatment program and provide trauma informed case management.
  • Maintain complete client records.
  • Prepare and present written status reports to the Family Court System regarding client's progress.
  • Investigate and arrange supportive services for clients.
  • Escort clients to treatment facilities when indicated transporting in own vehicle if necessary.
  • Maintain a caseload of substance abusing clients.
  • Complete all evidence-based assessments for appropriate assessment on clients
  • Make visits to treatment agencies assigned to caseload.
  • Identify and link clients to supportive services.
  • Have understanding of and refer to internal EAC programs, such as New Path, Health Home, CASA, Supervised Visitation among others.
  • Conduct urine, breath and hair follicle testing, maintain appropriate documentation and submit findings to court.
  • Install, monitor and maintain secure remote alcohol monitoring devices and submit findings to the court.
