As an RN Case Manager, you will collaborate with interdisciplinary healthcare teams across the continuum of care to coordinate seamless transitions and deliver high-quality, patient-centered care for adult neuro patients. In this role, you will apply your clinical expertise, critical thinking, and compassionate approach to advocate for patients and families while conducting holistic assessments, developing individualized care plans, and empowering patients to successfully self-manage their healthcare needs. Your work will play a vital role in promoting continuity of care, improving patient outcomes, and ensuring safe and effective transitions across healthcare settings. This neuro-specific position will be embedded directly within the nursing units of the neuro building, providing close collaboration with physicians, nursing staff, therapists, and other care team members. Candidates with prior neuro experience are strongly preferred, though previous case management experience is not required if supported by a strong neurological clinical background. Team members will complete a comprehensive 8–12 week onboarding and training program alongside experienced hospital case managers, including dedicated neuro-focused education and hands-on training designed to support long-term success within this specialized patient population.
