About The Position

Nourishing Hope is seeking a Bilingual (Spanish) Case Manager to support its social services programming. This role involves connecting individuals to essential resources such as transportation, housing, and benefits, engaging with stakeholders, tracking data, and ensuring program quality. The Case Manager will serve as a primary point of contact for clients, volunteers, and staff, upholding Nourishing Hope's core values of respect and dignity. The organization provides food, social services including case management and mental wellness counseling, and serves tens of thousands of people annually, with a significant number receiving case management services.

Requirements

  • Bachelor’s degree in social work or related human services field.
  • 1+ year of work experience in similar social services environments (clinical and case management setting preferred).
  • Bilingual (Spanish) required.
  • Strong technology skills including working knowledge of Microsoft Office Suite and Google Suite.
  • Ability to use and maintain Salesforce customer relationship management (CRM) data quality.
  • Commitment to Nourishing Hope’s mission and an understanding of and belief in the importance of food security for all.
  • Familiarity with the interconnected barriers experienced by neighbors experiencing food insecurity, including but not limited to homelessness, mental and physical health, disability, substance use, and aging-related barriers.
  • Strong interpersonal and communication skills (in person, over the phone, remotely, and in writing).
  • Ability to navigate challenging situations with patience and professionalism.
  • Proven ability to work independently and as a member of a team.
  • Willingness to provide staff coverage, depending on programmatic need.
  • Ability to prioritize workload and meet deadlines in a fast-paced environment with frequent interruptions.
  • A high level of professionalism, attention to detail, and a high degree of accuracy.

Nice To Haves

  • Master’s degree in social work or related human services field.
  • Previous experience in the hunger relief sectors.
  • Familiarity with the Chicago human services ecosystem.
  • Familiarity with trauma informed care principles.

Responsibilities

  • Support the execution of Social Services standard operating procedures (SOPs) to ensure high-quality programming, workplace safety, and supportive service standards.
  • Ensure adherence to organizational, state, and federal policy, and technology utilization and data quality.
  • Support the supervision of interns, as appropriate.
  • Act as the point of contact for all neighbors, volunteers, staff, and other stakeholders at food distribution sites and externally.
  • Ensure an environment that reinforces Nourishing Hope’s Core Values.
  • Ensure high-quality Social Services programming through outreach, scheduled appointments, and drop-in hours.
  • Coordinate access to supports related to healthcare, crisis intervention, domestic violence safety planning, financial assistance, transportation assistance, housing support, employment support, and benefits access.
  • Maintain accurate case records including assessments, interventions, service plans, and case notes in Azzly (EHR) and Salesforce.
  • Administer IM CANS assessments with Medicaid clients, as appropriate.
  • Actively participate as a member of the Mental Wellness & Social Services team.
  • Ensure cross-team and cross-departmental alignment with organizational priorities.
  • Participate in external meetings and trainings, as required.
