The Assertive Community Treatment (ACT) Case Manager serves as a member of a community-based mental health team that provides services for individuals experiencing serious mental illness. The mission of ACT is to help people become independent and integrate into the community as they experience recovery through a team-based service model. The ACT Case Manager will work with individuals and groups to promote optimum mental health. In addition, helps individuals deal with addictions and substance abuse, self-esteem, and issues associated with mental and behavioral health. As a community-based role, this position will render at least 80% of services in the beneficiary's home or other agreed-upon community-based position.
