The Southeast Health District, Coffee Wellness Clinic is seeking a highly qualified candidate to fill the full-time, salaried position of Case Manager Administrative Assistant based in Douglas, GA. Requirements

  • High school diploma/GED
  • Two (2) years of job-related experience.

Nice To Haves

  • Associate’s degree
  • Proficiency with Microsoft Office and electronic database or case management systems.
  • Strong customer service skills.
  • Experience in housing programs, social services, public health or human services.
  • Experience working with HIV/AIDS patients.
  • Bilingual

Responsibilities

  • Provides administrative and program support for the HOPWA program and related housing services.
  • Answers incoming calls, assists clients with housing-related inquiries, and provides direct support to case managers through client communications, documentation management, and case file maintenance.
  • Conducts intake assessments, annual reassessments, and recertifications to determine and maintain client eligibility.
  • Prepares and maintains accurate client records, and ensures all documentation complies with program and regulatory requirements.
  • Assists clients in accessing housing resources, including rental assistance programs, shelter services, and public housing opportunities.
  • Provides case management services for clients enrolled in supportive services-only programs.
  • Coordinates housing safety inspections.
  • Assists with HOPWA audits and monitoring activities.
  • Prepares required reports.
  • Tracks program compliance.
  • Supports special projects and other program activities to ensure the efficient delivery of housing assistance services.
