Requirements
- Graduate of an accredited school of professional nursing
- Active, unrestricted RN license in the state of practice
- 2–5 years of nursing experience
- 2–5 years of case management experience
- 2–5 years of administrative nursing experience
- Strong clinical assessment and critical-thinking skills
- Excellent communication and documentation abilities
- Professional, compassionate, and client-focused approach
Nice To Haves
- Experience in home care, assisted living, memory care, hospice, or senior services
- Dementia care experience
- Leadership or supervisory experience
- Passion for serving older adults and their families
Responsibilities
- Conducting comprehensive client assessments and initial nursing evaluations
- Developing, implementing, and updating individualized plans of care
- Coordinating services to ensure seamless client support
- Monitoring client progress and identifying changes in condition
- Educating clients, families, and caregivers on care needs and best practices
- Supervising and supporting caregivers to promote excellent client outcomes
- Assisting with medication management and health monitoring
- Reviewing and maintaining accurate clinical documentation
- Preparing nursing notes, visit summaries, and care updates
- Responding to urgent client, family, and caregiver concerns as needed
- Ensuring compliance with all state nursing regulations and ComForCare policies
- Serving as the clinical bridge between non-medical home care services and skilled nursing oversight
