About The Position

This position is responsible for interviewing, accepting, and providing structured case management services for an assigned caseload of clients participating in the Family Shelter program, with a focus on Social Services Benefits. The role involves determining social services benefits that clients are eligible for and guiding them through the application process. It requires understanding the uniqueness of each client's history to determine the most effective program plan, developing comprehensive program plans/goals, and evaluating client progress through weekly meetings. The position also ensures compliance with funding requirements.

Requirements

  • Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field, and three years progressively responsible experience
  • Or Any equivalent combination of training and experience which provides the required knowledge, skills, and abilities
  • Valid State Driver’s License
  • Ability to meet attendance requirements, which includes working weeknight evenings.
  • Ability to read, write, and communicate the English language effectively.
  • Ability to communicate clearly and effectively both orally and in writing.
  • Ability to think clearly and quickly to maintain control of client caseload.
  • Ability to effectively and efficiently work on multiple cases at the same time without becoming frustrated or disorganized.

Responsibilities

  • Ensures families participating in program are enrolled in social services benefits.
  • Understands the eligibility requirements and enrollment process.
  • Assists families with applying for appropriate social service benefits; becomes SOAR certified to assist with the process.
  • Outsources clients to additional counseling resources if needed; maintains awareness of progress with external case management professionals, working as a team to meet the holistic needs of the individual.
  • Meets regularly with clients to discuss and evaluate their progress, feelings, impressions, changes and personal growth/development; prepares accurate and up-to-date records documenting the same.
  • Serves as liaison to other agencies to obtain/coordinate assistance on client's behalf; builds and maintains professional working relationships with community agencies.
