About The Position

Matthew 25 is seeking a Case Management Specialist to provide medical case management services under the Kentucky HIV/AIDS Care Coordination Program (KHCCP). This role involves conducting social service assessments and connecting individuals living with HIV to essential services offered by Federal, State, and local community programs. The specialist will also assist the Housing Coordinator with housing programs, promote a Housing First approach, adhere to HOME TBRA and HOPWA guidance, and build relationships with property managers and leasing agencies.

Requirements

• A bachelor’s degree in social work, sociology, psychology, counseling, nursing, or related field is preferred. Candidates with case management experience of two years or more will also be considered as viable applicants.
• Experience working in a nonprofit organization or a strong desire to work in service driven environment.

• Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel
• Valid Driver’s License with at least three (3) years driving experience and less than two (2) moving violations in the past three (3) years.
• Must be able to pass a drug screen and background test.

Responsibilities

• Assist Housing Coordinator with Housing programs.
• Promote a Housing First approach with clients.
• Follow HOME TBRA and HOPWA guidance given by Kentucky Housing Corporation and HUD.
• Build relationships with property managers and leasing agencies.
