About The Position

As part of the Quality of Life by Choice model (“QLife”), conducts Case Management with clients in and around the QLife Program. Interviews, accepts, and provides comprehensive, long-term, structured, complex, case management services for an assigned caseload of clients participating in an established life management program (Street Teams/Comprehensive Treatment Court clients); understands the uniqueness of the client’s history in order to determine most effective program plans; develops comprehensive program plan/goals and evaluates client's progress by conducting mentoring and counseling sessions with client and/or staff; works with clients on client-centered plans with a focus on ending their homelessness through housing; serves as advocate for client in order to acquire services that will enable them to functionally cope with their environment and obtain housing.

Requirements

  • Bachelor's degree from an accredited college or university in Social Work, Behavioral Science, or a related field
  • Three years progressively responsible experience providing direct case management social services including accessing clients’ needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources.
  • Valid State Drivers License
  • Level 2 background screening

Responsibilities

  • Conducts Case Management and data entry associated with sheltered and unsheltered homeless persons seeking assistance within the QLife Shelter/Center of Hope complex.
  • Oversees case load of clients participating in The Salvation Army Street Team program collaboration along with the Comprehensive Treatment Court (CTC) program, serving homeless individuals with mental health and possibly co-occurring conditions.
  • Performs Coordinate Entry needs assessments for both the Street Team program and Shelter; Interviews applicants to determine eligibility for program enrollment based on established facility guidelines and requirements; conducts need assessment, obtain pertinent information; refers applicants to other agencies if not appropriate for program.
  • Conducts daily “sweeps” individually or in conjunction with Sarasota Police Department on 9th and Central advocating for those occupying the area to explore Salvation Army or Partner agencies programs; reminds neighbors of local ordinances.
  • Supervises clients while in the field when performing street cleaning duties; provides transportation when needed; ensures clients have access to water and/or other needed items.
  • Plans, coordinates, and/or facilitates life management, behavior modification, addiction-related, etc. classes to assist clients in obtaining skills that will enable them to functionally cope with their environment; obtains speakers and workshop presenters as needed.
