About The Position

This position is responsible for interviewing, accepting, and providing comprehensive, structured, complex case management services for an assigned caseload of clients participating in an established life management program, including domestic violence outreach, Pathway of Hope, and Prevention services. The specialist will understand the uniqueness of the client’s history to determine the most effective program plans, develop comprehensive program plans/goals, and evaluate client progress through mentoring and counseling sessions. The role also involves serving as an advocate for clients to acquire necessary services and ensuring compliance with funding requirements, including researching and obtaining funding for the Pathway of Hope program.

Requirements

  • Bachelor’s degree from an accredited college or university in Social Work, Behavioral Science, or a related field.
  • Three (3) years of progressively responsible experience providing direct case management and social services, including assessing clients’ needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources.
  • Valid State Driver’s License.
  • Ability to meet attendance requirements.
  • Ability to read, write, and communicate the English language.

Responsibilities

  • Interviews applicants to determine eligibility for program enrollment based on established facility guidelines and requirements.
  • Conducts needs assessments and obtains pertinent information; refers applicants to other agencies if not appropriate for the program.
  • Establishes unique comprehensive program goals (three months or more) for eligible clients based on personality, decision-making abilities, mental capabilities, addiction history, family involvement, etc.
  • Explains goals to clients in an easily understood manner and consistently counsels clients on ways to modify or stop negative behaviors while in the program.
  • Outsources clients to additional counseling resources if needed.
  • Maintains awareness of progress with external case management professionals, working as a team to meet the holistic needs of the individual.
  • Provides direct assistance in obtaining and maintaining self-sustaining sources of income, benefits, and other economic supports, as well as professional resources that enhance clients’ psychosocial well-being.
  • Meets regularly with clients to discuss and evaluate their progress, feelings, impressions, changes, and personal growth/development.
  • Prepares accurate and up-to-date records documenting client progress and interactions.
  • Receives incident reports and enforces disciplinary actions for infractions of house guidelines.
  • Facilitates individual and group discussions to ensure clients’ attitudes and motivations are progressing, redirecting negativity, promoting self-awareness, and providing appropriate encouragement.
  • Plans, coordinates, and/or facilitates life management classes to assist clients in obtaining skills to functionally cope with their environment, obtaining speakers and workshop presenters as needed.
  • Maintains awareness of program requirements necessary to maintain existing funding and ensures measurable outcomes correspond with requirements, making adjustments to case management processes as necessary.
  • Analyzes statistical information and case outcomes to ensure recidivism is not occurring regularly and recommends/implements changes to program guidelines to reduce repetitive requests for service.
  • Assists clients in locating/securing affordable housing and works closely with clients to transition into new housing.
  • Conducts home visitations when appropriate and ensures clients’ living quarters are properly furnished and maintained.
  • Contacts appropriate persons to facilitate needed repairs and conducts routine inspections of living quarters and grounds when applicable.
  • Refers clients to appropriate Corps programs or other agencies based on clients’ needs and in accordance with their program plan.
  • Completes and articulates discharge plans to clients exiting the program.
  • Prepares and maintains case records and logs on all assigned clients, ensuring accuracy and completeness.
  • Enters pertinent information into the established Homeless Management Information System (HMIS).
  • Attends community meetings with representatives of other agencies to educate them about the program, make client referrals, and maintain awareness of client progress.
  • Prepares and maintains statistical records on all services provided and complies/prepares monthly statistical reports, ensuring accuracy and completeness.
  • Prepares case presentations for the supervisor and presents cases directly to any Coalitions, funding bodies, or Case Conferences as directed.
