About The Position

Case Management Manager is responsible for staffing, planning and development for Case Management. Provides leadership, guidance, professional development and assistance to Case Management staff. Collaborates with nursing, medical staff, and other departments to ensure safe, quality, and compassionate discharge planning for positive patient outcomes.

  • BSN
  • BLS or CPR certification (within 90 days of hire)
  • RN-Registered Nurse - Illinois Department of Financial and Professional Regulation

Responsibilities

  • Analyzes quality improvement, risk management, and appropriate data for process improvement opportunities.
  • Assesses, plan, prioritizes, and monitors unit activities in order to provide assistance or serve as a resource to other staff members.
  • Assessment - Collects pertinent data and information relative to the patient's health or the situation. Recognizes normal and abnormal findings when gathering data.
  • Collaborates with staff to identify learning needs and provides staff with ongoing performance evaluation and feedback.
  • Collaborate with physicians and other disciplines to promote the best possible patient care through the process of mutual information sharing, discussion, and decision making.
  • Coordination of Care - Plans and evaluates care in collaboration with appropriate disciplines.
  • Develops in conjunction with the patient care team an appropriate and timely plan of care for the assigned caseload of patients.
  • Diagnosis - Analyzes assessment data to determine actual or potential diagnosis, problems, and issues.
  • Evaluates other team member as assigned using established criteria and goals. Participates in peer review process.
  • Evaluation - Evaluates progress toward attainment of goals and outcomes. Reports data and outcomes to others as appropriate.
  • Identifies learning needs of staff on team. Coordinates annual competency assessment. Follows-up with staff to assure standards are met. Initiates, develops, and presents educational program/activities (formal/informal)
  • Identifies potential risk to patients and others and initiates actions to minimize or prevent the risk.
  • Implementation - Implements care or work plan in alignment with the plan and approved safety, infection control, and department/organization standards.
  • Implements the role of preceptor according to Preceptor Accountabilities.
  • Knowledge - In collaboration with leaders, actively pursues required knowledge and skills through orientation activities specific to the position, reading current literature and seeking new learning opportunities.
  • Outcomes Identification - Identifies expected outcomes for a plan individualized to the patient or situations.
  • Participates in the identification of researchable projects through the utilization of unit quality improvement activities and trends and patterns related to nursing practice.
  • Participates in the ongoing variance monitoring of clinical pathways and implements action plans based on results of quality improvement activities which impact on patient care outcomes.
  • Planning - Develops a plan that prescribes strategies to attain expected, measurable outcomes.
  • Promote the delivery of safe, quality, compassionate care while maintaining a high standard of nursing practice.
  • Relationships - Establishes effective working relationships with peers, physicians, and other members of the health care / work team. Identifies and confers with appropriate resources regarding patient / work decisions.
  • Serves as a resource for clinical skills to staff. Evaluates staff performance of clinical skills.
  • Teaching - Employs teaching strategies to promote health and a safe environment.
