Requirements

• Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date.
• Social Worker credentialed from the Tennessee Board of Social Workers obtained prior to hire date or job transfer date.
• High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required.
• 3 years of experience required.
• 1 year of leadership or management experience required.

Nice To Haves

• Case Manager credentialed from the American Case Management Association (ACMA).
• Case Manager credentialed from the Commission for Case Manager Certification (CCMC).
• Master's degree preferred.

Responsibilities

• Manage assigned case managers, professional associates, and clerical staff to ensure productive workflows.
• Coordinate disease management programs and track patient outcomes throughout the post-acute period.
• Contract with internal and external healthcare resources to secure seamless transitions of care.
• Communicate regularly with administration and medical staff regarding admission appropriateness, length of stay (LOS), and discharge barriers.
