About The Position
The Case Manager provides services to inpatient, observation, emergency department, swing bed and resident home patients and is a patient advocate. This role assesses patients on admission for discharge needs and planning including home services, DME, short term and long term placement, financial assistance, emergency assistance, and psychiatric placement. Possess current working knowledge of third party payer guidelines and understanding of inpatient criteria.
Requirements
- Current nursing license, in good standing with the state licensing board
- CPR certification within 3 months of hire
