About The Position
The Case Management Care Coordinator assists in the clerical functions of the discharge planning process. This role involves the coordination, collection, and recording of Case Management information in compliance with Medicare, Medicaid, and third-party payers. The goal is to ensure discharge needs are met and that patients and their families understand the availability of resources for their medical and social support and education.
Requirements
- High School Diploma or GED equivalent
- Must be able to read, write, speak and understand English
- Computer operation skills
- Organizational skills
- Ability to communicate well
- Ability to prioritize
- Prior health care experience
