Position Summary:
We have an exciting opportunity to join our team as a Case Management Coordinator - Palliative Care - Full Time Days - Palliative Care - Full-Time - Monda.

In this role, the successful candidate The NYULH-Suffolk Palliative Care Team will provide patients with the highest quality medical care available, focusing on comfort and compassion. The team is committed to providing patient and family centered care that optimizes the quality of life by anticipating, preventing, and alleviating pain and suffering. A multidisciplinary and collaborative team approach model will be used, applying the professional core competencies of palliative care to include but not limited to; pain and symptom management, emotional, psychological, spiritual, family support, communication, and teaching.
The Palliative Care CM Coordinator will build a relationship with the patient, family and their support system, and will assist the patient and family in areas of need they have identified together.
This position will be responsible for departmental utilization review and CQI.

Requirements

• MSW
• Current New York State license to practice, LCSW preferred.
• Prior experience in palliative/hospice care/behavior health preferred
• Knowledge of federal, state and local regulations
• Experience in Quality Management, Utilization Review, Case Management
• Possesses current knowledge of CQI processes.
• Excellent verbal and written communication skills
• Must possess a New York State Drivers license.
• Lic Clinical Social Worker

Nice To Haves

• LCSW preferred
• Prior experience in palliative/hospice care/behavior health preferred

Responsibilities

• Assists patient and families in making healthcare decisions based on personal goals of care.
• Actively assists PC Team in development and completion of healthcare advance directives.
• Contacts local agencies / community resources that may be of help to patient and families.
• Assists with insurance, Medicare and Medicaid paperwork.
• Identifies emotional, behavioral, and spiritual needs of the patient and their loved ones, and coordinates appropriate supports needed.
• Assists with bridging family gaps to bring loved ones together whenever possible.
• Provides counseling for high-risk populations.
• Identifies other needs of the patient and their support circle and assists as required.
• Determines appropriate admission status, and need for continued stay, and assigns the appropriate level of care in collaboration with palliative care team.
• Completes Case Management Screen and Assessment according to hospital policy
• Identifies discharge planning needs where appropriate, and identifies barriers to timely discharge so as to assure effective and safe discharges.
• Effectively involves patient, family, care team, payors, and other resources in the discharge planning process.
• Promotes patient and family education regarding discharge options and available alternate care opportunities.
• Collects and analyses data to prepare effective and accurate reports
• Documents all interventions and findings.
• Provides timely and accurate plan and goals of care notes.
• Chart entries are clear, legible, concise, properly dated and signed.
• Maintain educational requirements to maintain certification.
• Actively seek educational opportunities in Palliative Care.
• Attend conferences and meetings as directed by the Palliative Care Physician.
• Establishes and develops performance improvement goals and objectives with the Palliative Care Physician and Nurse Practitioner.
• Monitors teams PI goals and objectives, and issues reports concerning outcomes (successes/ failures).
• Develops and maintains data collection for identified annual Performance Improvement Initiatives.
• Assures compliance with hospital wide PI initiatives.
• Attends hospital wide meetings concerning performance improvement, and represents the palliative care team in committee work.
• Communicates in a fashion that respects the individuality of each person on the team. Promotes dialogue that is non-confrontational and honest.
• Demonstrates a willingness to be flexible in working with others.
• Is an active and excited member of the team, and supports team goals.
• Easily collaborates with other team members, patients and families.

Benefits

• The salary range for the role is $70,282.50 - $105,439.67 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
