Care Coordinator - RN - Case Management - Hudson Valley Hospital - Full Time - Days
About The Position
Assist with the implementation of a Case Management Program. Provide leadership and education to other members of the Case Management and safe discharge planning strategies. This is a full-time position, working 4 shifts per week 7a-5p, at NewYork-Presbyterian Hudson Valley Hospital (located at Cortlandt Manor, Westchester County, NY). Rotating weekends are required. Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP. NYP will not reimburse for travel expenses.
Requirements
- BSN
- New York State RN license
- Ability to function independently and communicate effectively with administrative and professional staff and with people of diverse backgrounds and education.
- Ability to work with various people and make decisions about issues involving discharge planning, home health situations and available community services.
- Must possess good understanding of Managed Care/HMO requirements, laws and regulations, Medicare/Medicaid requirements, policy and procedure, and community health, welfare and social agencies policies and procedures.
Nice To Haves
- MS highly desirable.
- Minimum two years experience in Case Management.
- Certified Case Manager (CCM) highly desirable.
