About The Position
Rite of Passage Team is hiring for a Case Management Coordinator at Walton Academy for Growth and Change in DeFuniak Springs, FL. Walton Academy provides residential treatment to improve the lives of youth through mental health and substance abuse services, offering skills and opportunities from Sports to Culinary Arts. The Case Management Coordinator will have the unique opportunity to create a positive, safe, and supportive environment for the youth served while building a career rich in growth opportunities and self-fulfillment.
Requirements
- Bachelor’s degree in related field
- Four years of experience in Human Services
- Prior case management experience required
- Three years’ experience with juveniles
- Knowledge of Placing Agencies, licensing requirements and Rite of Passage policies and procedures.
- Ability to pass a criminal background check, drug screen, physical and TB test.
- Ability to effectively communicate, verbally and in writing.
- Must be able to maintain a high level of confidentiality.
- Able to work in excess of 40 hours per week as needed.
- Must be flexible and able to handle multiple priorities.
Nice To Haves
- Master’s degree preferred
- Two years of Rite of Passage experience
- Supervisory experience preferred
Responsibilities
- Overseeing all aspects of case management training, audit compliance and outside agency relationships, including community service, to ensure treatment continuity is maintained for the students.
- May supervise the Case Managers, Transitional Services or other designated staff.
